Parent Volunteer Clearances

Pennsylvania state law and Mellinger Mennonite Church policy require all parent volunteers of the Parent and Preschool Center to have proper clearances. 

If you have received these clearances for any other purposes during the past 5 years, you can email them to us or bring them to the preschool and we will scan them. If any of your clearances are expiring during this school year please update all three at this time. To volunteer for the full 2025-2026 School Year, your clearances will need be current through May 2026. Therefore, they would need to be dated after June 1, 2021.

Step 1: PA Criminal Record Check – No Cost. Valid for 5 years.

• Go to https://epatch.pa.gov/home website. 

• Click on New Record Check (Volunteers Only) and complete all fields

• Print the "Record Check Results Page" 

• IMPORTANT: Click on "CERTIFICATION FORM" above the BACK button to print your actual certificate. Submit this certificate to the preschool office. 

Step 2: Child Abuse History Clearance – No Cost. Valid for 5 years.

• Go to https://www.compass.dhs.pa.gov/cwis/public/home. 

• Click "Create Individual Account," if you do not already have a Keystone ID

• Complete all fields

• Select: Volunteer

• Volunteer Category: Other

• Agency Name: Mellinger Mennonite Church Parent and Preschool Center

• Print out the certificate. Write down or remember your login and password to access the certificate at any time. 

Step 3: Select one of the following:

• If you have been a PA resident for 10 consecutive years: Disclosure Statement Application for Volunteers. No cost.  

  • Print the form here, sign and submit to the preschool office.

• If you have NOT been a PA resident for 10 consecutive years: FBI Criminal History Record. Cost $22.60. Valid for 5 years.  

  • Go to https://www.identogo.com/locations/pennsylvania to access the website.

  • Select Digital Fingerprinting option at the bottom of the page

  • Enter the Service Code: 1KG6ZJ

  • Click on Go

  • Complete Registration and Schedule an appointment at a fingerprint collection site.

  • This fee is paid at the fingerprint site.  Submit results to preschool office.

Step 4: Acknowledgement on Child Protection Policy Form. No Cost. 

• Read and review:

  • Policy: Part 1 Sec. D2 & Sec. E./ Part 3 Sec. A. & Sec. K

  • Policy: Appendix A

• Print, sign and submit the form found here.

Step 5: Compile all completed clearance documentation and submit to the preschool office- or email jonelle@mellingerchurch.org. Keep a copy of your clearances for your records.
